Every person or clinical laboratory licensed or registered under this chapter shall report to the department, within 30 days thereof, any change of name or address.

Requires every person or clinical laboratory licensed or registered under the chapter to report any change of name or address to the department within 30 days of the change. The obligation applies to both individual persons and clinical laboratories holding chapter credentials and states no alternative reporting period or exception.
