The application shall be verified and shall include the following:
(a) The full name, residence address, telephone number, and date of birth of the employee.
(b) The name, address, telephone number, and license number of the employer and the date the employment commenced.
(c) A statement as to whether the employee has been convicted of a misdemeanor, excluding minor traffic violations.
(d) A statement as to whether the employee has been convicted of a felony.
Notes
Requires a verified alarm-agent registration application to include the employee’s full name, residence address, telephone number, and birth date; the employer’s name, address, telephone number, license number, and employment start date; and statements disclosing whether the employee has misdemeanor convictions other than minor traffic violations or any felony conviction.
