The certificate shall contain the name of the person to whom issued. Proper index and record of each certificate shall be kept by the board.

Notes

Requires each architecture certificate to state the name of the person to whom the certificate is issued. The board must also keep a proper index and record of every certificate, creating both an identification requirement for the certificate itself and a corresponding board recordkeeping duty for issued certificates.
