An application for a branch office registration shall include:
(a) The full name and address of, and the telephone number at, the principal licensed location, and the license number of the applicant.
(b) The address of, and the telephone number at, the branch office.
Notes
Requires an application for registration of a branch office to state the full name, address, telephone number, and license number of the principal licensed location. Also requires the application to provide the branch office’s address and telephone number, identifying both the existing licensed location and proposed branch.
