Any person may obtain blank license application forms from the board or may cause to be printed forms used by or approved by the Registrar of Contractors.

Notes

Allows any person to obtain blank contractor-license application forms directly from the Contractors State License Board. Also allows a person to have application forms printed, provided the printed forms are the forms used by or approved by the Registrar of Contractors rather than an unapproved alternative form.
