The notice of a claim of lien shall be filed on a form prescribed by the Secretary of State pursuant to Section 9502 of the Commercial Code. The standard form shall be completed in its entirety except as follows:
(a) The lien claimant may be identified either as a lien claimant or as a secured party.
(b) The form shall be signed by the lien claimant and need not be signed by the lien debtor.
(c) The description of the collateral shall be the information specified in subdivisions (c), (d), (e), and (g) of Section 22909.
(d) Attached to the form shall be a separately signed statement containing the information specified in subdivision (f) of Section 22909.
Notes
Requires an equipment-repurchase lien claim notice to use the Secretary of State's prescribed financing-statement form and be completed fully except for specified modifications. Allows the claimant to be identified as lien claimant or secured party, requires only the claimant's signature, prescribes the collateral description, and requires an attached separately signed statement containing the referenced perjury information.
