The application shall contain the following information:

(a) The name of the applicant.

(b) For a general partnership, the names of the individual partners.

(c) For a limited partnership, limited liability company, or a corporation, the name of the entity.

(d) The location of the premises for which the license is applied.

Notes

Requires a license application to state the applicant name and the premises location, and to identify individual partners for a general partnership or the entity name for a limited partnership, limited liability company, or corporation. These listed identity and location items are the information the provision expressly requires.
